Preparing for Your FAA Medical Exam

Prior to your appointment, you’ll need to complete Items 1–20 of FAA Form 8500-8 through the FAA’s MedXPress system.

• Access the system at: medxpress.faa.gov

• After submission, you’ll receive:

  • A confirmation number

  • A printable summary page

• Be sure to enter your home address when completing the application

• Your MedXPress application remains valid for 60 days

• Your medical exam cannot be conducted until this step is completed

 

What to Bring With You

• Your MedXPress confirmation number

• A government-issued photo ID that verifies both identity and age

  • Acceptable forms include: driver’s license, state ID, passport, or military ID

• If you plan to use a certified birth certificate for age verification, you must also bring a separate photo ID

• Third Class: $300

• Second Class: $400
